California Medical Provider Network (MPN) and Employee Information

Dear Policyholder:

The Division of Workers Compensation in California amended its regulations regarding Medical Provider Networks (MPN) and employee information effective August 27, 2014. As part of the amendment, there are some changes to the process.

Here’s what you need to know:

  • The new law requires that the full MPN written notification is required at the time of injury, when there is knowledge of an injury or when an employee with an existing injury is required to transfer treatment to an MPN (9767.12). This notification can be provided electronically to the injured employee, in writing at the time of injury or when the employee is being transferred into the MPN. The employee should sign the acknowledgment.
  • The MPN requirements have been simplified to allow for all employers choose the MPN. Formal implementation, letters and posting of Employee Notification documents is no longer required.
  • Employees reporting a claim need to be provided with the Employee MPN Notification Packet, with a new "Unique MPN Number" affixed to the document. This document is specific to the Chubb Underwriting Company.

  • A Medical Provider Network Medical Access Assistant is available to help injured employees with finding available MPN physicians and scheduling provider appointments. The Medical Access Assistant line for Chubb is 1-855-857-7556 and is available from 7am to 8pm Monday through Saturday. The fax line is 866-708-4331.
  • The Notice to Employees (DWC-7) should include the MPN information if you are participating in the program.
  • The updated New Hire Pamphlet is to be provided to each new employee at the time of hire. This includes facts about the MPN, Workers’ Compensation facts, Pre-Designation of Personal Physician and Notice of Personal Chiropractor/Acupuncturist.

As before, the Workers Compensation Claim Form (DWC-1) along with the Notice of Potential Eligibility should be completed with each employee at the time of injury. The DWC-1 and signed acknowledgment should be provided when reporting a claim.

To find medical providers use the Medical Access Assistant line or use the following instructions access the online CorVel MPN:

  • Go to "http://www.corvel.com"
  • Click "PROVIDER LOOK-UP"
  • Click "Find a Provider"
  • Select a Network "California MPN"
  • Enter your zip code
  • Under Specialty, select "Occupational Medical Center", then click "Find Providers"
  • This will provide you with a list of medical clinics in your area that are part of the MPN

Participation in the MPN will help support medical cost containment and facilitate prompt return to work. If you do not already participate in the MPN and are interested in participating, please notify Chubb prior to implementation so that you will be in compliance.
