Report Your Claim

1.   Download the Work Injury Compensation Insurance claim form.

2.   Submit your Chubb Work Injury Compensation Claim Form along with a copy of the following supporting documents.

Required Claims Supporting Documents

  •  
    • A copy of the iReport submitted to the Ministry of Manpower (MOM)
    • Final medical bills and receipts
    • Medical Certificates
    • Doctor’s memo for outpatient medical treatment
    • Inpatient discharge summary / medical report (for hospitalisation of injured employee
    • Salary payment vouchers (12 months prior to the accident)
    • Work Permit or Employment Pass of injured employee (for foreign employees)
    • Death Certificate / Autopsy Report (for fatalities)
    • Police report or incident report (for road traffic accidents)

Please note that above list is not exhaustive, and we may request for additional documents on a need-be basis.

Our hotline operating hours are from Mondays to Fridays, 9.00am – 5.00pm, excluding Public Holidays. 

 

How long will it take to process my claim?

For straightforward claims, upon receipt of your claim and all the supporting documents, we will finalise our claims adjudication within 7 business days.

If your claim is accepted and you have opted for electronic funds transfer, we will reimburse you within the next 5 business days.

If your claim is accepted and you have opted for cheque payment, we will send you the cheque within the next 7 business days.
