1. Through CIMB Niaga Bank Savings account. Please complete required documents below:
   • Form of BizChannel Authorization Form (applicable for Syariah and Non-Syariah products) which provided by CIMB Niaga Bank at Chubb Life offices.
   • Copy of valid ID card and copy of front page savings account book/clearing account
2. Through BCA Bank Savings account. Please complete required documents below:
   • Form of Surat Kuasa Pendebitan Otomatis Rekening BCA (Applicable for Syariah Products) or Surat Kuasa Pendebitan Otomatis Rekening BCA (Applicable for Non-Syariah Products)
   • Copy of valid ID card and copy of front page savings account book/clearing account
3. Through Mandiri Bank Savings account. Please complete required documents below:
   • Form of Aplikasi Kuasa Pendebetan Mandiri 
   • Copy of valid ID card, copy of front page savings account book/clearing account, and copy of ATM card
4. Through Visa or Mastercard Credit Card. Please complete required documents below:
   • Form of Surat Kuasa Pendebitan Otomatis Kartu Kredit (Applicable for Syariah Products) or Surat Kuasa Pendebitan Otomatis Kartu Kredit (Applicable for Non-Syariah Products)
   • Copy of valid ID card and copy of credit card
