Documents required for processing of accident insurance claims:

  • Company's Claim Request Form Part 1, the insured to complete 
  • Company's Claim Request Form Part 2, the doctor to complete, or medical certificate 
  • Copy of the national ID card or other identity documents issued by the Government, with the signature to certify 
  • Copy of the police report, if any 
  • X-ray film or a copy of x-ray report in the event of a bone fracture or dismemberment
  • The insured's consent letter to disclose medical treatment history

Note: The Insured's signature on the Claim Request Form must be the same as in the insured's policy document.
