Getting Claim Forms
- Visit our Customer Care Center at Chubb Life Assurance, or call 1283, from Monday to Friday between 08.30 and 17.00 hrs. Our officer will give/send the Claim Request Form to you
- Contact one of Chubb Life Assurance's branch offices throughout Thailand
- Contact your servicing agent
- Download the Claim Form from our website
Submission of Claim Documents
- Submit the documents to Chubb Life Assurance's head office or branch offices throughout Thailand
- Submit the documents via registered mail to : Claims Department, Chubb Life Assurance Public Company Limited 130-132 Sindhorn Building, Tower 3, 22nd Floor, Wireless Road, Lumpini, Pathumwan, Bangkok 10330
- Submit the documents to your servicing agent
Claim Time Frames
- The company will pay the Insured/beneficiary according to the policy's coverage conditions within 15 days of the company receiving the completed claim documents.
- Should the claim be in doubt, i.e. the claim is not in accordance with the policy's coverage conditions, the company will extend the payment to 90 days to make provisions to investigate the claim further.
